STATE BOARD OF NURSING RELEASES DISCIPLINARY ACTIONS

HARRISBURG (June 16) - Commonwealth Secretary Kim Pizzingrilli and Dorothy Childress, commissioner of the Bureau of Professional and Occupational Affairs today announced 45 disciplinary actions taken by the Pennsylvania State Board of Nursing between March 1 and April 30, 1999.

Actions were taken against individuals in the following counties: Allegheny (five actions); Armstrong (one action); Beaver (one action); Berks (one action); Blair (one action); Cambria (one action); Chester (one action); Clearfield (one action); Crawford (one action); Cumberland (one action); Dauphin (one action); Delaware (one action); Erie (one action); Fayette (two actions); Lancaster (one action); Lebanon (two actions); Luzerne (two actions); Lycoming (one action); Montgomery (three actions); Montour (one action); Philadelphia (two actions); Warren (one action); Washington (one action); Wayne (one action); Westmoreland (two actions; and York (one action). Eight actions involved Pennsylvania licenses with out-of-state addresses. 

The State Board of Nursing:

SUSPENDED Janice Rosporski, license no. RN-118309-L, of Pittsburgh, Allegheny County, for three years retroactive to Feb. 8, 1999, for violating a board order. (March 1);

REVOKED Mary J. Ott, license no. PN-075172-L, of Pittsburgh, Allegheny County, retroactive to Oct. 22, 1998, for violating a board order. (March 3);

SUSPENDED Barry L. Peck, license no. PN-069528-L, of Altoona, Blair County, retroactive to Oct. 22, 1998, for violating a board order. Peck's suspension shall continue until he documents successful completion of a course in patient rights or elder abuse pursuant to the order dated Nov. 17, 1998. (March 3);

INDEFINITELYSUSPENDED Beth Ann Phillips, license no. RN-207777-L, of Mount Laurel, New Jersey, retroactive to Feb. 8, 1999, for violating a board order. (March 3);

AUTOMATICALLY SUSPENDED Janet Addison, license no. PN-072245-L, of Grampian, Clearfield County, based on her guilty plea to a felony violation under the Controlled Substance Act. (March 3);

AUTOMATICALLY SUSPENDED Lynette J. Eisaman, license no. RN-230986-L, of Irwin, Westmoreland County, based on her guilty plea to 26 felony violations under the Controlled Substance Act. (March 3);

AUTOMATICALLY SUSPENDED Kristin Zierle Lynn, license no. RN-326211-L, of Cogan Station, Lycoming County, based on her guilty plea to a felony violation under the Controlled Substance Act. (March 4);

REPRIMANDED Steven C. Gottshall, license no. RN-260001-L, of Palmyra, Lebanon County, based on disciplinary action taken against his license in another state. (March 8);

Accepted the VOLUNTARY SURRENDER of the license issued to Audrey Boyd, license no. RN-159414-L, of Ambler, Montgomery County, based on her impairment. (March 8);

SUSPENDED Migdalia Gracia, license no. PN-093574-L, of Lancaster, for three years based on her impairment. The suspension was stayed in favor of probation with participation in the disciplinary monitoring program. (March 8);

A Probable Cause Screening Committee SUSPENDED Craig A. Stumpf, license no. PN-089367-L, formerly of Harrisburg, Dauphin County, for three years for violating the terms of his consent agreement. The consent agreement had found Stumpf to be unable to practice with reasonable skill and safety to patients by reason of his addiction to drugs and alcohol, and had provided for a three-year suspension of his license that was stayed in favor of a three year probation in a voluntary recovery program for impaired practitioners. (March 9);

SUSPENDED Phyllis Forcina, license no. RN-253680-L, of Philadelphia, for two years, based on her impairment. The suspension was stayed in favor of probation. (March 9);

SUSPENDED Gary L. Bryington, license no. PN-151229-L, of Drums, Luzerne County, for three years retroactive to Jan. 27, 1999, for violating a board order. (March 9);

SUSPENDED Heather L. Cochran, license no. RN-313085-L, of Philadelphia, for three years, retroactive to Dec. 7, 1998, for violating a board order. (March 9);

A Probable Cause Screening Committee SUSPENDED Joseph D. Engle, license no. RN-266627-L, of Reading, Berks County, for three years for violating a board order. (March 10);

A Probable Cause Screening Committee SUSPENDED Gary Lee Chapman, license no. PN-253052-L, of Washington, for three years for violating a board order. (March 10);

REVOKED Elizabeth A. Fuhrmann, license no. RN-280261-L, of Danville, Montour County, based on her guilty plea in Colorado to attempting to obtain a controlled substance by fraud and deceit.  
(March 15);

SUSPENDED Lisa Niebling Barry, license no. RN-314851-L, of Nixa, Missouri, based on her inability to practice because of dependence on narcotic drugs and alcohol. The suspension shall remain in effect until she can demonstrate that she has passed a three year period free from alcohol and drugs. 
(March 15);

SUSPENDED Lee Robinson Novick, license no. RN-319706-L, of Pittburgh, Allegheny County, for five years, based on disciplinary action taken against her license in New York. Because she presented significant mitigating evidence, the suspension was stayed in favor of probation retroactive to March 13, 1996, and to coincide with her New York suspension period. (March 16);

AUTOMATICALLY SUSPENDED Denise Doyle Mammarelli, license no. RN-290110-L, of Norwood, Delaware County, because she pled guilty to possession with intent to deliver Schedule I and Schedule II controlled substances. (March 23);

AUTOMATICALLY SUSPENDED Patrice MorganWoods, license no. RN-275470-L, of Camp Hill, Cumberland County, because she pled guilty to 26 counts of obtaining a Schedule III controlled substance through forgery and/or fraud. (March 23);

AUTOMATICALLY SUSPENDED Sally A. Graff, a/k/a Sally Terry, license no. RN-156829-L, of Kittanning, Armstrong County, based on her guilty plea to obtaining a controlled substance by theft. (March 26);

SUSPENDED Tamara L. Kroplesky, license no. PN-256858-L, of Honey Brook, Chester County, for three years retroactive to Feb. 8, 1999, because she violated a board order. (April 6);

SUSPENDED Kathleen Flaherty Behlke, license no. PN-081810-L, of Honesdale, Wayne County, for two years and nine months, retroactive to Feb. 8, 1999, because she violated a board order. (April 8);

AUTOMATICALLY SUSPENDED Kelly Betters Geissler, license no. RN-309358-L, of West Homestead, Allegheny County, based on her guilty plea to two misdemeanor violations of the Controlled Substance Act. (April 15);

AUTOMATICALLY SUSPENDED Renee Heeter Grosch, a/k/a Renee M. Grosch, license no. RN-317814-L, of Clarendon, Warren County, based on her guilty plea to a felony violation of the Controlled Substance Act. (April 15);

A Probable Cause Screening Committee SUSPENDED Susan A. Rumball, license no. RN-297609-L, of Erie, for three years for violating the terms of her probation. (April 19);

INDEFINITELY SUSPENDED Robert Brennan, license no. RN-193496-L, of Blounts Creek, NC, based on disciplinary action taken against his license in North Carolina for falsifying information on a patient's medical record. (April 19);

A Probable Cause Screening Committee SUSPENDED Roberta Ceriani Ciotti, license no. RN-330264-L, of Conneautville, Crawford County, for violating the probationary terms of a previous order which had found her to be unable to practice with reasonable skill and safety to patients by reason of her addiction to drugs, and had provided for a three year suspension of her license that was stayed in favor of a three year probation in a voluntary recovery program for impaired practitioners. (April 19);

A Probable Cause Screening Committee SUSPENDED Robert P. Lukaszewicz, license no. RN-331801-L, of Aliquippa, Beaver County, for three years for violating a board order. (April 19);

A Probable Cause Screening Committee SUSPENDED Jenny M. Friedman, license no. RN-319775-L, of Colmar, Montgomery County, for three years for violating a board order. (April 19);

Accepted the VOLUNTARY SURRENDER of the RN and PN licenses issued to Michelle R. King, license nos. RN-307387-L, and PN-075080-L, of Johnstown, Cambria County, based on her inability to practice nursing with reasonable skill and safety to patients by reason of mental illness and physiological or psychological dependence on alcohol. (April 19);

INDEFINITELY SUSPENDED Michelle S. Graff, license no. RN-306646-L, of Cinnaminson, New Jersey, based on her impairment. The suspension was stayed in favor of probation until Oct. 25, 2001. (April 19);

SUSPENDED Robin Evans McDonald, license no. RN-345052-L, of Port Byron, New York, for three and one half years, based on her conviction for a crime of moral turpitude and her being disciplined by the New York State Board. (April 19);

REPRIMANDED Phyllis Santiago, license no. PN-026573-L, of Norristown, Montgomery County, and ORDERED her to complete continuing education, for failing to document and maintain accurate patient records. (April 19);

SUSPENDED June Wilson, license no. PN-028726-L, of Westampton, New Jersey, for one year for providing her employer with a forged medical examination form. The suspension was stayed in favor of probation, and she was ordered to complete a course in nursing ethics. (April 19);

INDEFINITELY SUSPENDED Nadene R. West, license no. RN-193919-L, of Greensburg, Westmoreland County, because of her guilty plea to the felony charge of criminal attempt based upon her alteration of a prescription. Her suspension shall continue until she provides documentation to the board that she is physically and mentally fit to practice and free of any drug or alcohol impairment. (April 20);

ORDERED that the license issued to Kimberly Stevens Sumey, no. PN-093064-L, of Uniontown, Fayette County, be placed on probation for 30 months because of her criminal felony convictions for which she was sentenced to 30 months criminal probation and ordered to pay a $1,600 fine. (April 20);

INDEFINITELY SUSPENDED Terry Grimm Scott, a/k/a Terry L. Scott, license no. PN-102813-L, of Dover, York County, because of her addiction to drugs and/or alcohol. She may petition the board for reinstatement after at least three years of active suspension. If reinstatement is granted, she shall be placed on probation for at least two years. (April 20);

SUSPENDED Arthur M. Masteller, license no. RN-240526-L, of Big Pine Key, Florida, for 30 months, retroactive to June 24, 1997, for violating the terms of his probation. (April 20);

A Probable Cause Screening Committee SUSPENDED Patricia Chilson, license no. RN-294819-L, of Topeka, Kansas, for violating her previous board order. (April 20);

A Probable Cause Screening Committee SUSPENDED Jennifer Brightwell Bitonti, license no. RN-313044-L, of Belle Vernon, Fayette County, for violating the terms and conditions of a board order. (April 20);

A Probable Cause Screening Committee SUSPENDED Pauline Calabrese, license no. PN-096935-L, of West Pittston, Luzerne County, for violating the terms and conditions of a board order. (April 20); 

A Probable Cause Screening Committee SUSPENDED Diane M. Ebbitt, license no. RN-288851-L, of Pittsburgh, Allegheny County, for violating the terms and conditions of a board order. (April 20); and

REVOKED David P. Formigoni, license no. RN-323189-L, of Annville, Lebanon County, for committing immoral or unprofessional conduct and possessing, using and distributing a controlled substance for other than an accepted medical usage. (April 21).
