State Board of Nursing Release Actions - (August 20, 2002)

HARRISBURG (Aug 20) – Commonwealth Secretary C. Michael Weaver and David Williams, Acting Commissioner of the State Department’s Bureau of Professional and Occupational Affairs today announced 109 disciplinary actions taken by the State Board of Nursing between January and June, 2002.

Actions were taken against individuals or firms in the following counties: Allegheny (14 actions); Beaver (2 actions); Berks (1 action); Blair (1 action); Bucks (5 actions);  Cambria (2 actions); Centre (2 actions); Chester (1 action); Clinton (2 actions); Crawford (1 action); Cumberland (2 actions); Delaware (7 actions); Elk (1 action); Erie (3 actions); Fayette (3 actions); Huntingdon (1 action); Lackawanna (1 action); Lancaster (3 actions);  Lebanon (1 action); Lehigh (1 action); Luzerne (5 actions); Lycoming (2 actions); McKean (1 action); Mercer (1 action); Montgomery (7 actions); Montour (1 action); Northampton (1 action); Northumberland (1 action); Philadelphia (9 actions); Union (1 action); Warren (2 actions); Washington (3 actions); Westmoreland (6 actions); York (6 actions). State Board of Nursing Releases Disciplinary Actions

 

Wendy A. Randolph, LPN, license no. PN-086079-L, of Millmont, Union County, suspended her license for one year, with six months stayed in favor of probation, based on findings that she engaged in unprofessional conduct. (01-02-02)

Ann L. Bittner, RN, license no. RN-204394-L, of Beaver Falls, Beaver County, was given a public reprimand and levied a civil penalty in the amount of $1,000.00, based on her practicing practical nursing while her license was expired or lapsed. (01-02-02)

Brenda M. Burkey, LPN, license no. PN-073036-L of Hamburg, Berks County, PA was assessed a civil penalty of $500 and a Public Reprimand placed  on her Board record based upon her violating the Board’s regulations. Burkey engaged in the practice of nursing while her license was lapsed. Burkey last practiced in Hamburg, PA.  (1/10-11/02)

Tami Doll Eynon, license no.  PN-093478-L, of Dillsburg, York County, imposed a public reprimand and $800.00 civil penalty because she practiced practical nursing without a current license.  Respondent last practiced in Mechanicsburg, PA.  This Order becomes effective immediately. (01-10-02)

Louis M. Konkol, license no. RN-257384-L, of Erie, Erie County, indefinitely suspended for no less than 3 years, such suspension stayed in favor of probation subject to various terms and conditions the professional nurse license, based on his inability to practice the profession with reasonable skill and safety by reason of illness, addiction to drugs or alcohol, or mental impairment.  Respondent last practiced in Erie, PA.  This Order becomes effective immediately. Kristin Lynn Bosko, license no. PN-253859-L of Pittsfield, Warren County, issued a civil penalty of $750.00 based on her practicing nursing on a lapsed license from June 30, 1988 until February 2000. (01-10-02)

 

Alyson R. Gill, license no. RN-327488-L, of Clinton, NJ, automatically suspended the license to practice professional nursing based on her guilty pleas to four felony counts under New Jersey law, which if committed in Pennsylvania, would be felonies under the Controlled Substance, Drug, Device and Cosmetic Act. (01-23-02)

 

Jack M. Macey, license no. RN-228628-L, of Jeannette, Westmoreland County, was reinstated, subject to two years of monitored probation. (01-23-02)

 

Jade Thoms Incelli, license no. RN-284272-L, of Levittown, Bucks County, was suspended for three (3) years, based upon her violation of the January 8, 1998 Consent Agreement and Order of the Board of Nursing placing her on probation and requiring her to participate in the Professional Health Monitoring Programs.  Incelli violated the Order by failing to abstain from the use of prohibited drugs.   (02-08-02)

 

Celia A. Wire, license number RN-188303-L, of Dover, York County, had the stay of the suspension of her license vacated, the period of probation terminated and her license to practice nursing actively suspended for no less than 3 yrs, based on her violation of a previously issued Board Order. (02-08-02).

 

Denise Young, license numbers RN-333741-L and PN-101421-L, of Williamsport, Lycoming County, had the stay of the suspension of her license vacated, the period of probation terminated and her license to practice nursing actively suspended for no less than 3 yrs, based on her violation of a previously issued Board Order. (2-8-02)

 

Jade Thoms Incelli, license no. RN-284272-L, of Levittown, Bucks County, was suspended for three (3) years, based upon her violation of the January 8, 1998 Consent Agreement and Order of the Board of Nursing placing her on probation and requiring her to participate in the Professional Health Monitoring Programs.  Incelli violated the Order by failing to abstain from the use of prohibited drugs.   (02-08-02)

 

Gary Brinton, R.N., license no. RN-192108-L, of Bainbridge, Lancaster County, was assessed an $800.00 civil penalty and a public reprimand, based on practicing professional nursing on an expired license. (02-08-02)

 

Elisa Marie Hensler, R.N., license no. RN-500008-L, of Verona, Allegheny County, was assessed an $800.00 civil penalty and a public reprimand, based on practicing professional nursing on an expired license. (02-08-02)

 

Jodi Phillips, license number RN-507882-L, of New Alexandria, Westmoreland County, was actively suspended for at least 3 years, based on her violation of a previously issued Board Order. (02-08-02).

 

Christian D. Gagliardi, R.N., license no. RN-509607-L, of Philadelphia, Philadelphia County, was imposed an indefinite suspension stayed in favor of no less than three years probation, based on Respondent being unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment.  (02-8-02)

 

ANNA M. PELOT, license no. RN-247195-L, of Drums, Luzerne County, was suspended for five years for practicing professional nursing while her license was suspended and for committing fraud or deceit in the practice of nursing.  (1-15-02)

 

PATRICIA A. SELLERS, license no. RN-172405-L, of Havertown, Delaware County, was denied her application for reinstatement based on findings that she had not demonstrated that she was fit to practice.  Her license has been suspended since October 30, 1996 because she had violated a drug and alcohol treatment and monitoring agreement.  The Board also suspended her license for two years based on disciplinary action imposed by the Kentucky Board of Nursing.   This case has been appealed to Commonwealth Court.  (1-17-02)

 

JESSICA N. HUMPHREYS, license no. RN-528905-L, of York, York County, was placed on no less than three years probation subject to terms and conditions, following a stay of the Board’s automatic suspension issued on May 22, 2001, based on her violation of the Controlled Substance, Drug, Device, and Cosmetic Act.   (01-15-02)

 

NANCY LUGO, unlicensed, of Philadelphia, Philadelphia County, was ordered to cease the unlicensed practice of practical nursing and assessed a civil penalty in the amount $2,000, based on her alteration of a Pennsylvania Department of Health Certificate to indicate that she was licensed as a practical nurse. (1-15-02)

 

F. DARLENE HARRIS BUCKNER, license no. PN-072363-L, of Pittsburgh, Allegheny County, was revoked based on her conviction of one felony count of insurance fraud.   (01-23-02)

 

Brian T. Hasselman, R.N., license no. RN-515877-L, of St. Marys, Elk County, was temporarily suspended for no more than 180 days on the grounds that his continued practice within the Commonwealth may make him an immediate and clear danger to the public health and safety based upon his addiction to drugs.   (03-07-02)

 

Kathy Godfrey, LPN, license no. PN-093568-L, of Pittsburgh, Allegheny County, was issued an indefinite suspension for no less than three (3) years, such suspension to be immediately STAYED in favor of no less than three (3) years of PROBATION, based on her being unable to practice the profession of nursing with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. Kimberly Truver Riggin, LPN, license no. PN-100735-L, of Greensburg, Westmoreland County, was issued a civil penalty of One Thousand Dollars ($1,000), based on her practicing on a lapsed practical nursing license. (03/07/02)

 

MARGARET WHITMAN PURCELL, license no. PN-099598-L, of Cleona, Lebanon County, was assessed a $1,000 civil penalty based on Respondent continuing to practice nursing on an expired license. (3-7/8-02)

 

Troy A. Sorchilla, R.N., license no. RN-517858-L, of Johnstown, Cambria County, was assessed a civil penalty of $800.00 and a public reprimand, based on Respondent practicing professional nursing without a valid, current license. (03-7/8-02)

 

Christina M. Ruggirello, L.P.N., license no. PN-258921-L, of Downingtown, Chester County, was assessed an $800.00 civil penalty and a public reprimand, based on practicing practical nursing without a valid current license. (03-07/08-02)

 

Grayce Gambaro Talarico, R.N., license no. RN-266850-L, of Pittsburgh, Allegheny County, was indefinitely suspended for no less than 3 years, such suspension to be immediately stayed in favor of no less than 3 years probation, based upon Respondent being unable to practice professional nursing with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. Michael T. Greene, R.N., license no. RN-316134-L, of Erie, Erie County, accepted the voluntary surrender of his professional nursing license, based on Respondent being unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. (03-7/8-02)

 

Kimberly A. Bailey, license no. RN-213329-L, of Newville, Cumberland County, was assessed a civil penalty of $1,000.00 and had a public reprimand placed on her record for practicing without a license from November 1998 through March 2001. (3-7&8-02)

 

George G. O’Gorman, license no. RN-310605-L, of Shickshinny, Luzerne County, was reinstated and placed on a three-year period of probation retroactive to April 27, 2000.  (2-14-02)

 

PATRICIA J. MIESEL, license no. RN-331873-L, of Eighty Four, Washington County, was suspended for three years, retroactive to October 26, 2001, based on findings that she violated the terms and conditions of the Voluntary Recovery Program Consent Agreement and Order.  (3-25-02)

 

JANSON WILLIAMS, license no. PN-250461-L, of Erie, Erie County, was suspended for three years retroactive to September 13, 2001, based on findings that he violated the terms and conditions of the Voluntary Recovery Program Consent Agreement and Order.  (3-25-02)

 

ADAM V. CRAIN, license no. PN-262398, of Philipsburg, Centre County, was granted admission to the practical nursing examination and upon his successful completion of the examination, a license to practice practical nursing will be issued and immediately suspended until the applicant submits to a drug and alcohol evaluation and assessment and the Board has reviewed that evaluation, based on the applicant’s guilty pleas to two misdemeanor counts under the Controlled Substance, Drug, Device and Cosmetic Act.  (2-19-02)

 

LAURIE A. VALDISERRI, license no. PN-105971-L, of Belle Vernon, Fayette County, was suspended for three years, retroactive to October 26, 2001, based on findings that she violated the terms and conditions of the Voluntary Recovery Program Consent Agreement and Order.  (2-22-02)

 

ROBIN LOUISE COOK, license no. RN-212616-L, of Philadelphia, Philadelphia County, was suspended for three years, retroactive to October 27, 2000, based on her violation of the terms of her Voluntary Recovery Program Consent Agreement and Board Order.  (02-12-02)

 

HEATHER OAKLEY JOHNSTON, license no. RN-286622-L, of Beaver Falls, Beaver County, was indefinitely suspended retroactive to October 31, 2000 based on her violation of the terms and conditions of her Voluntary Recovery Program agreement and Board order. (02-02-2002)

 

MARY JANE VINCH, license no. RN-343844-L and certification no. SP-004229-B, of Lawrenceville, NJ, was suspended indefinitely, stayed in favor of probation to run concurrently with probation imposed by the New Jersey State Board of Nursing, based on disciplinary action taken by the New Jersey State Board of Nursing.  (2-21-02)

 

MARTHA LEATHERS WESTON, license no. RN-244794-L, of Russell, Warren County, was revoked for findings that she had been disciplined in New York.  (2-13-02)

 

RAE GRENINGER SNOOK, license no. PN-082488-L, of Williamsport, Lycoming County, was automatically suspended for her conviction of the felony charge of the acquisition or obtaining possession of a controlled substance by misrepresentation, fraud, forgery, deception or subterfuge.  (3-01-02)

 

MARGIE LEH RONDOSH, license no. PN-104502-L, of Hazleton, Luzerne County, was indefinitely suspended until such time as she submits to a mental and physical examination as directed by the State Board of Nursing and it is determined that she is able to practice practical nursing with reasonable skill and safety to patients.  (3-12-02)

 

LORRAINE E. GLICKMAN, license no. PN-253696-L, of Elkins Park, Montgomery County, was automatically suspended based on her conviction of a felony under The Controlled Substance, Drug, Device and Cosmetic Act.  (3-05-02)

 

KIMBERLY C. WHITE, license no. PN-255652-L, of Philadelphia, Philadelphia County, was suspended for 21 months, retroactive to May 22, 2000, based on findings that she violated her probation with the Board.  (3-26-02)

 

COLEEN MARTIN CONLON, license no. RN-259379-L, of Leisenring, Fayette County, was indefinitely suspended due to her failure to submit to a mental and physical examination as directed by the Board’s Probable Cause Screening Committee.  (3-15-02)

 

KELLY BETTERS GEISSLER, license no. RN-309358-L, of West Homestead, Allegheny County, was indefinitely suspended retroactive to September 18, 2001 because she violated the terms of her Voluntary Recovery Program Consent Agreement and Board Order.  (3-27-02)

 

Robin Mignogna Jorgensen, R.N., license no. RN-327987-L, of Ridley Park, Delaware County, was assessed an $800.00 civil penalty, based on practicing professional nursing on an expired license. (04-08-02)

 

Kathryn Boone, R.N., license No. RN-216103-L, of Newtown Square, Delaware County, permanently surrendered her license to practice nursing based on her suffering from a chemical impairment that prohibits her from practicing nursing with reasonable skill and safety. (04-08-02)

 

LAURA M. DEARDORFF, license no. PN-106740-L, of Hanover, York County, was suspended for one year, with one month active suspension and the remainder stayed in favor of probation, based on unprofessional conduct and misappropriation of money from a patient  (4-10-02)

 

JANE STYPERK FRIEND, license no. PN-082429-L, of Houston, Washington County, was suspended for one year, with 45 days active suspension and the remainder stayed in favor of probation, based on unprofessional conduct and misappropriation of money from a patient  (4-10-02)

 

SHERI LYNN HECKLER, license no. PN-253655-L, of Lancaster, Lancaster County, was indefinitely suspended until she submits to a mental and physical examination as directed by the Board. (4-10-02)

 

SHARON STANICH HOUSEHOLDER, license no. RN-274505-L, of Port Vue, Allegheny County, was suspended for three years, retroactive to September 13, 2001, based on her violation of the terms and conditions of her Voluntary Recovery Program agreement and Board order.

REGINA MILLS HUMPHREYS, license no. PN-104063-L, of Pittsburgh, Allegheny County, was suspended for two years, retroactive to May 2, 2001, with the suspension stayed in favor of probation beginning on May 10, 2002, based on her failure to timely complete a course in medication administration pursuant to the terms and conditions of a consent agreement and Board order.  (4-10-02)

 

VALERIE MARTIN JAGOE, license no. RN-349506-L, of Lancaster, Lancaster County, was revoked because she had her license revoked by the proper licensing authority of another state.  (4-10-02)

 

CYNTHIA LOU PITKAVISH, license no. PN-250493-L, of Conneaut Lake, Crawford County, and Northville, MI, was automatically suspended for up to one year based on findings that she was convicted of a misdemeanor under the Pennsylvania Controlled Substance, Drug, Device and Cosmetic Act.  (4-24-02)

 

THOMAS CHARLES SENICK, license no. RN-508471-L, of Pittsburgh, Allegheny County, was indefinitely suspended based on findings that he violated the terms and conditions of the Voluntary Recovery Program Consent Agreement and Order.  (4-10-02)

 

TAMMY G. ADKINS, license no. RN-262597-L, of Mt. Pleasant, Westmoreland County, was suspended for three years, retroactive to March 7, 2002, because she violated the terms of her Voluntary Recovery Program Consent Agreement and Board Order. (4-10-02)

 

LAURA J. GOWEN, license no. RN-316105-L, of Hatfield, Montgomery County, was suspended for three years, retroactive to March 7, 2002, because she violated the terms of her Voluntary Recovery Program Consent Agreement and Board Order.  (4-10-02)

 

LINDA JULIANO, license no. RN-505746-L, Bear, DE, was suspended for three years, retroactive to March 7, 2002, based on her violation of the terms and conditions of her Voluntary Recovery Program agreement and Board order.  (4-10-02)

 

BEVERLY STROUP DITTMAN, license no. RN-262246-L, of Lewis Run, McKean County, was reinstated to probation for a period of at least three years, based on her successful completion of the terms of her indefinite suspension under a previous Board order.  (4-10-02)

MONIQUE WATERS GUNN, license no. RN-229043-L, of Shippensburg, Cumberland County, was suspended retroactive to March 7, 2002 based on violation of the terms and conditions of her Voluntary Recovery Program consent agreement and Board order.  (4-24-02)

 

JESSICA N. HUMPHREYS, license no. RN-528905-L, of York, York County, was suspended for three years, retroactive to March 7, 2002, based on violation of the terms of a Board Order directing her participation in the Disciplinary Monitoring Unit of the Professional Health Monitoring Program. (4-24-02)

 

AMBER RADAKER, license no. RN-524667-L, of Glassport, Allegheny County, was suspended for three years, retroactive to March 28, 2002, based on findings that she violated the terms and conditions of her Voluntary Recovery Program Consent Agreement and Order.  (4-29-02)

 

KIMBERLY MANNING, license no. PN-252829-L, of Uniontown, Fayette County, is permitted to take the Professional Nurse Examination and upon successful completion of that exam shall have that professional nursing license granted, but shall be placed on probation to run concurrent with the probationary period of her practical nursing license.  (3-22-02)

 

CELIA A. WIRE, license no. RN-188303-L, of Dover, York County, was actively suspended for three years retroactive to February 13, 2002, based on findings that she violated the terms and conditions of her Voluntary Recovery Program Consent Agreement and Order.  (3-27-02)

 

JADE THOMS INCELLI, license no. RN-284272-L, of Levittown, Bucks County, was suspended for three years, retroactive to February 8, 2002, based on her violation of the terms of her Voluntary Recovery Program Consent Agreement and Board Order. (3-27-02)

 

TERRI A. CASALE, license no. RN-304886-L, of Loretto, Cambria County, was granted admission to the practical nursing examination and upon her successful completion of the examination, a license to practice practical nursing will be issued and immediately placed on probation for one year, based on the applicant’s guilty plea to a crime of moral turpitude.  (2-12-02)

 

Starina Ann Norris Burket, L.P.N., license no. PN-102111-L, of Alexandria, Huntingdon County, accepted the voluntary surrender of her professional nursing license, based on Respondent being unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. Keely M. Benson, R.N., license no. RN-329907-L, of Glenshaw, Allegheny County, accepted the voluntary surrender of her professional nursing license, based on Respondent being unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. (05-16/17-02)

 

Mileva W. Kempa, LPN, license no. PN-252164-L, of Pittsburgh, Allegheny County, has voluntarily surrendered her practical nursing license for at least 3 years, based on Respondent being unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. Monica Ann Maxon, L.P.N., license no. PN-253026-L, of Vestaburg, Washington County, accepted the indefinite suspension of her practical nursing license for no less than three years, such suspension to be immediately stayed in favor of no less than three years of probation, based on Respondent being unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. Donna Roberts Munsell, license no. PN-078126-L, of Hatboro, Montgomery County, was assessed a public reprimand and $1,000.00 civil penalty, because she practiced nursing while her license was not currently renewed.  (5-16-02)

 

Julia T. Bonjo, license no. RN-155219-L, of King of Prussia, Montgomery County, was assessed a public reprimand and $2,000.00 civil penalty, because she pled guilty to a felony and was guilty of immoral or unprofessional conduct. (5-16-02)

 

Deborah Bradley, R.N., license no. RN-202307-L, of Brookhaven, Delaware County, was assessed a $750.00 civil penalty, based on Respondent practicing nursing on a lapsed license. (05/16/02)

 

Janet M. Samberg, license no. PN-107181-L, of Easton, Northampton County, was issued a $1,000.00 civil penalty along with a public reprimand on her permanent Board record, based on Respondent practicing practical nursing on a lapsed license. (4/16/02)

 

KOFI A. FRIMPONG, license no. RN-340571-L, of Maplewood, NJ, was revoked, based on the suspension of his nursing license by the proper licensing authority of New Jersey.  (5-17-02)

 

DIANE M. KOETHE, license no. RN-289799-L, of Pittsburgh, Allegheny County, was suspended for three years, retroactive to April 8, 2002, based on her violation of the terms and conditions of her Voluntary Recovery Program agreement and Board order.  (5-13-02)

 

DEBRA ZIMMERMAN HUTKA, license no. RN-296385-L, of Laughlintown, Westmoreland  County, was suspended for three years, retroactive to April 8, 2002, based on her violation of the terms of her Voluntary Recovery Program Consent agreement and Board order. (05-13-02)

 

SHERRY A. HARVEY, license no. RN-316249-L, Archbald, Lackawanna County, was suspended for three years, retroactive to April 8, 2002, based on her violation of the terms and conditions of her Voluntary Recovery Program agreement and Board order.  (5-13-02)

MARGARET TAYLOR EHGARTNER, license no. RN-261462-L, of Altoona, Blair County, was automatically suspended based on her guilty plea to a felony under the Controlled Substance, Drug, Device and Cosmetic Act.  (05-24-02)

 

LORI KRISTEN KREUGER, license no. RN-315316-L, of Pittsburgh, Allegheny County, was suspended for at least three years, retroactive to April 9, 2002, based on her violation of the terms and conditions of her Voluntary Recovery Program agreement and Board order.  (05-15-02)

 

TAMI M. LEH, license no. PN-107099-L, of Pottstown, Montgomery County, was suspended for at least three years, retroactive to April 10, 2002, based on her violation of the terms and conditions of her Voluntary Recovery Program agreement and Board order.  (5-15-02)

KATHY NATALE CANGEMI, license no. RN-330159-L, of Tempe, AZ, was denied reinstatement of her professional nursing license, based on her failure to present information to the Board concerning her sustained rehabilitation and treatment.  (05-17-02)

 

LYNN HOWARD-RYDER of Philadelphia, Philadelphia County, was granted admission to the practical nursing examination and upon her successful completion of the examination, a license to practice practical nursing will be issued and immediately placed on probation concurrent with applicant’s criminal probation, based on the applicant’s guilty pleas to misdemeanor crimes of moral turpitude.  The applicant was denied a temporary practice permit to practice as a graduate practical nurse.  (05-17-02)

 

TYRONE COOK, license no. RN-292752-L, of Philadelphia, Philadelphia County, was revoked, based on disciplinary action taken against his license by the proper licensing authority of another state and deceit in renewing his Pennsylvania license. (05-22-02)

 

JENNIFER ROSS MORRISON, license no. RN-313494-L, of Bellefonte, Centre County, was suspended for three years, retroactive to April 8, 2002, based on findings that she violated the terms and conditions of the Voluntary Recovery Program Consent Agreement and Order.  (05-22-02)

 

JOANNE S. WELTON, license no. PN-055634-L, of Renovo, Clinton County, was revoked for practicing as a practical nurse while her license was suspended, and failed to provide information concerning her employer and supervisor in accordance with the terms of her prior probation.  (05-23-02)

 

JODI ANN PHILLIPS, license no. RN-507882-L, of New Alexandria, Westmoreland County, was indefinitely suspended based on findings that she violated the terms and conditions of the Voluntary Recovery Program Consent Agreement and Order.  (05-23-02)

 

PAUL F. NARCOONIS, license no. PN-059885-L, of Wilkes-Barre, Luzerne County, was indefinitely suspended based on findings that he violated the terms and conditions of the Voluntary Recovery Program Consent Agreement and Order.  (05-23-02)

 

BARBARA BELLIS, license no. RN-315084-L, of Philadelphia, Philadelphia County, was revoked, based on practicing while her license was suspended, misappropriation of drugs from an employer and use of a controlled substance for other than an acceptable medical purpose. (05-22-02)

 

Eileen Lawrence Germani, license numbers RN-514852-L and PN-106257-L, of Doylestown, Bucks County, was permanently surrendered, based on her being unable to practice the profession with reasonable skill and safety to patients by reason of dependence on narcotic drugs and being guilty of unprofessional conduct by altering a prescription written by her physician.  (6-24-02)

 

Angela J. Shipley, license numbers RN-358924-L and PN-253610-L of Weirton, West Virginia, permanently voluntarily surrendered her nursing licenses, because she pled guilty to a felony or crime of moral turpitude in a court of this Commonwealth.  (6-24-02)

 

Paul R. Thomas, license no. RN-279194-L, of King of Prussia, Montgomery County, voluntarily surrendered his license, based on his inability to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment.  (6-24-02)

 

Catherine Antkowiak, R.N., license no. RN-302458-L, of Philadelphia, Philadelphia County, was indefinitely suspended for no less than three years, such suspension to be immediately stayed in favor of no less than three years of probation, based on Respondent being unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. Heather L. Williams, L.P.N., license no. PN-256564-L, of Shamokin, Northumberland County, was assessed an $800.00 civil penalty and a public reprimand, based on Respondent practicing practical nursing without a valid current license. (06-24-02)

 

Nancy J. Lulay, license no. RN-533422-L, of Claymont, Delaware, voluntarily surrendered her nursing license because she is unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment. (6-24-02)

 

MARY GOTTLIEB, license no. RN-186920-L, of Scottsdale, AZ, was issued a public reprimand, based on the imposition of a civil penalty on her nursing license by the proper licensing authority in Arizona.  (6-27-02)

 

CRYSTAL HENSEL LUNSFORD, license no. PN-150731-L, of Rising Sun, MD, was indefinitely suspended, based on her guilty plea to a felony and crime of moral turpitude.  (06-27-02)

 

LORRAINE STICKELMEYER, license no. RN-275809-L, of West Middlesex, Mercer County, was indefinitely suspended, to run concurrent with her criminal probation and until at least April 15, 2003, based on findings that she pled guilty to a felony in Ohio. (06-27-02)

 

JOSEPH A. RACEMUS, license no. PN-095626-L, of Plains, Luzerne County, was suspended for two years, with at least six months of active suspension and the remainder stayed in favor of probation based on findings that he committed fraud or deceit in the practice of practical nursing and engaged in unprofessional conduct.  (06-27-02)

 

CHRISTOPHER R. RICE, license no. PN-099581-L, of Morton, Delaware County, was revoked based on findings he pleaded guilty to a felony in the courts of this Commonwealth.  (06-27-02)
