STATE BOARD OF NURSING RELEASES DISCIPLINARY ACTIONS 

HARRISBURG (April 2) ­ Acting Commonwealth Secretary Kim Pizzingrilli and Dorothy Childress, commissioner of the Bureau of Professional and Occupational Affairs today announced 62 disciplinary actions taken by the Pennsylvania State Board of Nursing between Jan. 1 and Feb. 28, 1999. 

Actions were taken against individuals or firms in the following counties: Adams (one action); Allegheny (eight actions); Beaver (three actions); Berks (two actions); Bucks (three action); Butler (one action); Chester (four actions); Columbia (one action); Crawford (one action); Delaware (one action); Elk (one action); Fayette (one action); Lancaster (three actions); Lehigh (one action); Luzerne (three actions); Lycoming (one action); McKean (one action); Monroe (one action); Montgomery (one action); Northumberland (one action); Philadelphia (four actions); Schuylkill (one action); Tioga (one action); Washington (three actions); Wayne (two actions); Westmoreland (two actions); and York (three actions). Seven actions involved a Pennsylvania license with an out-of-state address. 

The State Board of Nursing: 

SUSPENDED Barbara Walsh, license no. RN-158639-L, of Allegheny County, for three years for violating her consent agreement. (Dec. 23); 

SUSPENDED Catherine A. Cooper license no. PN-063200-L, of Orwigsburg, Schuylkill County, for two years, retroactive to Dec. 7, 1998, for violating her consent agreement. (Jan. 5); 

REVOKED Rachel A. Flury, license no. PN-062024-L, of Emigsville, York County, retroactive to Dec. 7, 1998, for violating her consent agreement. (Jan. 5); 

SUSPENDED Pamela K. Cook, license no. RN-289850-L, of Jeanette, Westmoreland County, for up to one year, for pleading nolo contendere to possession of a controlled substance. (Jan. 5); 

DENIED the application of Timothy L. Smith, of Camden, NJ, for a certificate to practice as a licensed practical nurse, retroactive to Oct. 22, 1998, based on his guilty plea in NJ to possession of a controlled dangerous substance with intent to deliver within 1,000 feet of a school. (Jan. 5); 

SUSPENDED Patricia Loll-Van Sickle, license no. RN-343424-L, of Elmer, NJ, for three years for violating her consent agreement. (Jan. 5); 

SUSPENDED Scott J. Amrozewicz, license no. RN-349022-L, of Exton, Chester County, for two years for violating a board order. (Jan. 7); 

SUSPENDED Tracy Thomas Pace, license no. RN-254882-L, of Lititz, Lancaster County, for three years for violating a board order. (Jan. 7); 

SUSPENDED Neal G. John Jr., license no. RN-336552-L, of Aliquippa, Beaver County, for three years for violating his consent agreement. The consent agreement had found John to be unable to practice with reasonable skill and safety to patients by reason of his addiction to alcohol, and had provided for a three-year suspension that was stayed in favor of a three years probation in a voluntary recovery program. (Jan. 7); 

SUSPENDED Gary L. Bryington, license no. PN-151229-L, of Drums, Luzerne County, for three years for violating his consent agreement. The consent agreement had found Bryington to be unable to practice with reasonable skill and safety to patients by reason of his addiction to alcohol, and had provided for a three-year suspension that was stayed in favor of a three years probation in a voluntary recovery program. (Jan. 7); 

SUSPENDED Donna Fleming Altland, license no. PN-151229-L, of York Haven, York County, for three years for violating her consent agreement. The consent agreement had found Altland to be unable to practice with reasonable skill and safety to patients by reason of his addiction to drugs, and had provided for a three-year suspension that was stayed in favor of a three year probation in a voluntary recovery program. (Jan. 7); 

SUSPENDED Jack Macey, license no. RN-228628-L, of Latrobe, Westmoreland County, for one year for failing to document and maintain accurate records. The suspension was stayed in favor of probation. (Jan. 7); 

SUSPENDED Deborah Conway Bullard, license no. RN-315497-L, of Gibsonia, Allegheny County, for three years based on physiological or psychological dependence upon alcohol or drugs. The first six months of the suspension are active. The remainder was stayed in favor of probation. (Jan. 7); 

SUSPENDED Pamela Dunn Orshaw, license no. RN-285242-L, of Northumberland, for seven years, for her guilty plea to felony charges and her inability to practice with reasonable skill and safety to patients by reason of mental illness. The suspension was stayed in favor of probation. (Jan. 7); 

accepted the VOLUNTARY SURRENDER of the license issued to Barbara Hoover Snavely, license no. RN-267241-L, of Manheim, Lancaster County, because she is unable to practice with reasonable skill and safety to patients because of her impairment. (Jan. 7); 

SUSPENDED Monica Irwin Ream, license no. PN-070489-L, of Eldred, McKean County, for three years for failing to obey a previous board order. The suspension was stayed in favor of probation. (Jan. 8); 

SUSPENDED Susan Ulshafer Bensinger, license no. RN-308495-L, of Reading, Berks County, for five years, because of her impairment. The suspension was stayed in favor of probation. (Jan. 8); 

INDEFINITELY SUSPENDED Larissa Hlad, license no. RN-231286-L, of Carnegie, Allegheny County, because of her impairment. The suspension was stayed in favor of probation. (Jan. 8); 

ORDERED Carol Rae, license no. RN-144986-L, of Wilkes Barre, Luzerne County, to pay a $250 civil penalty for practicing on a lapsed license. (Jan. 8); 

SUSPENDED Debbra Chynoweth Scotti, license nos. RN-506034-L and PN-073935-L, of Lansdale, Montgomery County, for three years based on immoral or unprofessional conduct. Four months of the suspension are active. The remainder was stayed in favor of probation. (Jan. 8); 

REVOKED Rochelle H. Ferguson, license no. PN-106801-L, of Sciota, Monroe County, based on her conviction. (Jan 12); 

INDEFINITELY SUSPENDED Trina Smith, license no. RN-320725-L, of Butler, because of her chemical dependency and for violating the terms of her Voluntary Recovery Program agreement. After three years, she may petition the board for reinstatement. If reinstatement is granted, she will be placed on probation for at least two years. (Jan .12); 

SUSPENDED Mary Lynn Tarbuck, license no. RN-257260-L, of Washington, PA, for five years because of her chemical dependency and for violating her Voluntary Recovery Program agreement. The first three months are active. The remainder was stayed in favor of probation. (Jan. 12); 

REVOKED Anthony Pasquariello, license no. RN-293776-L, of Washington, PA, because his license was revoked in another jurisdiction. (Jan. 12); 

DENIED Marcella R. Norman, license no. RN-324785-L, of Monaca, Beaver County, reinstatement of her license until she can demonstrate three years of sobriety. Ms. Norman's license was previously suspended on March 4, 1997, based on her convictions under the Drug Act. (Jan. 12); 

DENIED Jennifer Mitchell of Reading, Berks County, reinstatement of her license after she failed to appear for her reinstatement hearing, and ordered that before she may be issued any future licenses, she must come before the board for a formal hearing to prove that she possesses all the statutory requirements to be licensed in PA. Her license was previously revoked on Jan 14, 1998, for committing fraud or deceit in the practice of nursing after pleading guilty to a criminal charge of altering records. (Jan .12); 

SUSPENDED Donald M. Nar, license no. RN-506852-L, of Honesdale, Wayne County, for forty-two days followed by probation because he filed a false report to law enforcement authorities. The suspension is retroactive to Nov. 6, 1997, and the probationary period will run concurrently with his criminal probation which is to continue until May 1999. (Jan. 12); 

INDEFINITELY SUSPENDED Peter A. Dorsaneo, license no. PN-098833-L, of Clifton Heights, Delaware County, and Escondido, CA, because of drug abuse. His license shall remain suspended until he can demonstrate at least three years of documented recovery. (Jan. 12); 

SUSPENDED Linda Galanos, license no. RN-169867-L, of Levittown, Bucks County, for one year, followed by a two year probation, for possessing and acquiring a controlled substance for other than an acceptable medical purpose and for misappropriating drugs from her employer. (Jan. 13); 

REVOKED Cynthia Ferguson, license no. RN-152624-L, of Williamsport, Lycoming County, based on her guilty plea to a felony offense of retail theft. (Jan. 13); 

ORDERED Donna M. Murray, license no. RN-300480-L, of Downingtown, Chester County, to be placed on probation for four years under chemical abuse guidelines because of her chemical dependency and her violation of the Voluntary Recovery Program agreement. (Jan. 14); 

SUSPENDED Lucia C. Warmkessel, license no. PN-079665-L, of Lehigh County, for two years for violating a previous board order. (Feb. 8); 

SUSPENDED Janice Rosporski, license no. RN-118309-L, of Allegheny County, for three years for violating a previous board order. (Feb. 8); 

SUSPENDED William J. Randal, license no. PN-107338-L, of Allegheny County, for three years for violating a previous board order. (Feb. 8); 

INDEFINITELY SUSPENDED Marilyn Santiago McMullin, license no. RN-349266-L, of Asheville, NC, for violating a previous board order. (Feb. 8); 

SUSPENDED Tamara L. Kroplesky, license no. PN-256858-L, of Chester County, for three years for violating a previous board order. (Feb. 8); 

SUSPENDED Kathleen Flaherty Behlke, license no. PN-081810-L, of Wayne County, for two years and nine months for violating a previous board order. (Feb. 8); 

SUSPENDED Beth Ann Phillips, license no. RN-207777-L, formerly of Philadelphia, for three years for violating a previous board order. (Feb. 8); 

INDEFINITELY SUSPENDED Vernice Pryor Tull, license no. PN-256343-L, of Philadelphia, for violating a board order. The suspension was stayed in favor of probation. (Feb. 8); 

SUSPENDED Janice L. Demma, license no. RN-245028-L, of Aliquippa, Beaver County, until March 31, 2000, and ORDERED her to complete a course in pharmacology, for a misdemeanor violation of the Controlled Substance, Drug, Device and Cosmetic Act, and for administering medication prior to obtaining a physician's order. The suspension was stayed in favor of probation. (Feb. 8); 

SUSPENDED Sandra Capezzuto Malinoski, license no. RN-316908-L, of Pittsburgh, Allegheny County, for three years for repeated acts of negligence or incompetence in the practice of nursing. The first year of the suspension is active. The remainder was stayed in favor of probation. (Feb. 8); 

INDEFINITELY SUSPENDED Lisa Dugan Kotler, license no. RN-295747-L, of Atlanta, GA, for pleading guilty to a felony in another state. (Feb. 8); 

SUSPENDED Jonni J. Chutes, license no. RN-300549734-Y, of Gettysburg, Adams County, for two years for being unable to practice with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol or mental incompetence. The suspension was stayed in favor of probation. (Feb. 8); 

SUSPENDED Linda Bald Pittman, license no. PN-097502-L, of Philadelphia, for a minimum of three years, because she pled guilty to one felony count of welfare fraud and one felony count of medical assistance fraud. (Feb. 9); 

SUSPENDED Kathleen A. Smith, license no. PN-074337-L, of Johnsonburg, Elk County, for a minimum of three years, because of incompetence, repeated violations of the law and the board's regulations, and for unprofessional conduct. (Feb. 9); 

SUSPENDED Donna Fleming Altland, license no. PN-103561-L, of York Haven, York County, for three years retroactive to Jan. 7, 1999, for violating the terms of her consent agreement. (Feb. 9); 

SUSPENDED Kathleen W. Gennuso, license no. RN-318365-L, of Cecil, Washington County, for three years retroactive to Nov. 17, 1998, for violating the terms of her consent agreement. (Feb. 9); 

SUSPENDED Neal G. John Jr., license no. RN-336552-L, of West Liberty, WV, for three years, retroactive to Jan. 7, 1999, for violating the terms of his consent agreement. (Feb. 9); 

ORDERED Ruth Vandermark Allen, license no. PN-095027-L, of Philadelphia, to be placed on probation until Oct. 30, 1999, for violating a previous board order. (Feb. 11); 

ORDERED Susan Haeberle Ritorto, license no. RN-307587-L, of Fairless Hills, Bucks County, to pay a $500 civil penalty because she failed to report disciplinary action taken in another state. The disciplinary action taken in the other state expired without further violations, and Ms. Ritorto presented significant mitigating evidence. (Feb. 11); 

ORDERED the license of Christina Winterling, license no. RN-326115-L, of West Chester, Chester County, to be reinstated subject to a three year probation. After three years, she may petition the board to have her license reinstated to unrestricted, non-probationary status upon verification that her unmonitored practice does not present a threat to public health and safety. Ms. Winterling's license was indefinitely suspended on May 7, 1996. (Feb. 11); 

INDEFINITELY SUSPENDED Peter J. Gallagher, license no. PN-080837-L, of Pittsburgh, Allegheny County, for drug and alcohol abuse. The suspension will remain in effect until he can demonstrate three years of documented recovery. (Feb. 12); 

REPRIMANDED Charlene Ann Santana, license no. PN-252815-L, of Philadelphia, and ORDERED her to complete six continuing education credits regarding methods of dealing with combative patients because she inappropriately dealt with an abusive patient. If Ms. Santana does not complete the continuing education credits, her license will be suspended. (Feb. 16); 

REPRIMANDED Julaine A. Owlett, license no. PN-092098-L, of Nelson, Tioga County, based on disciplinary action taken against her license in another state. (Feb. 16); 

INDEFINITELY SUSPENDED Anna M. Pelot, a/k/a Anna Pelot Yackanicz , license no. RN-20185-L, of Drums, Luzerne County, but not for less than two years, because of her continued alcohol and drug abuse. (Feb. 16); 

SUSPENDED Karen E. Shuman, license no. RN-194290-L, of Charleston, SC, for six months, based on disciplinary action taken against her license in another state. The suspension was stayed in favor of a two year probation. (Feb. 16); 

REPRIMANDED Stacey C. Weisser, license no. PN-256121-L, of Bloomsburg, Columbia County, because she attempted to divert drugs for her personal use. (Feb. 16); 

INDEFINITELY SUSPENDED Diane Stetar, license no. PN-043025-L, of North Versailles, Allegheny County, but for not less than three years, based on her addiction to drugs and for willfully misappropriating drugs from her employer. (Feb. 16); 

REVOKED Mary Beth Davis, license no. RN-189962-L, of Uniontown, Fayette County, based on her felony convictions and crimes of moral turpitude. (Feb. 16); 

SUSPENDED Tracy Thomas Pace, license no. RN-254882-L, of Lititz, Lancaster County, for three years retroactive to Jan. 7, 1999, because she violated the terms and conditions of her probation. (Feb. 17); 

INDEFINITELY SUSPENDED Darlene Brenot, license no. PN-100084-L, of Venango, Crawford County, but for not less than three years, based on drug and alcohol abuse, conviction of a crime of moral turpitude, incompetence, misappropriation of drugs and unprofessional conduct. (Feb. 18); and 

REVOKED David J. Adams, license no. RN-329687-L, of River Ridge, LA, for violating the terms of a consent agreement. (Feb. 23).
